3. Broadbridge Heath Junior Football Club Terms and Conditions

At times the club may wish to take photos or videos of the team or individuals in it. We adhere to the FA Guidelines to ensure that these are safe and respectful and solely for the purposes for which they are intended, which is promotion and celebration of club activities and for training purposes.

Travel

Arrangements will be made to collect and return children to specific pick up points. The Club's responsibility stops and starts at these points. It is your responsibility to arrange safe travel to and from the pick-up points. If you are ever delayed in collecting your child, please make every effort to contact the Club Contact or the escorting helpers so we can discuss arrangements for your child.

Non-Football Activities

There may be occasions when your child is involved in planned and structured non-football activities, such as a trip to the cinema, bowling or participating in a fund raising event. The club will have collected appropriate information in preparation for the activity, identified any potential risks or dangers, minimised the risks and dangers by careful planning and precautions and made sure we know who is responsible for putting precautions into place.

Remote Supervision

There may be occasions when your child is involved in planned and structured unsupervised non-football activities (e.g. shopping trip) whilst on tours. Players will only be permitted to participate in these activities if it is considered appropriate to do so. You will be informed prior to the event if any form of remote supervision will take place and the nature of the activity.

Insurance

It is the responsibility of you or the player on this form to arrange medical insurance if you feel the level of cover provided by the club is not adequate. The club provides basic cover for broken bones only.

Playing For The Team

I understand that by the way of Club Membership, I am not automatically entitled to play in any competitive football as entered by the club. Team selection is by the Team Manager only and his decision is FINAL. I also agree to be bound by and to observe the club rules and the rules and regulations of the F.A. Ltd and County FA and all competitions in which the club participates.

I understand that prior to joining the club, I have settled all financial liabilities with any other club, League and, or the FA (including the return of any kit or equipment).

Parental Agreement

In the event that my child is injured whilst training / playing football / travelling to and from football matches and I cannot be contacted, I hereby agree that my child can receive medical attention and grant permission for a qualified member of the coaching staff to provide emergency 1st aid if required.

Parental Agreement

I agree that I will not at any time leave my child unattended and if I nominate an appropriate adult, such adult will be advised to the team manager.

Consent Of Parent/Legal Carer

I give consent for my child to participate in Broadbridge Heath JFC events and agree to the conditions outlined above. I accept that it is my responsibility keep the details held in Spond uptodate.
